I was planning my trip on the app, but one trip that I found with the trip planner online does not appear in the app. So I wanted to add it manually, but I was wondering how I should fill it in with a trip that has a transfer. Do I add the different trains individually or just from the beginning city to the last city?

Correct, you will need to add every train separately.
The mobile FAQ doesn't say it explicitly but since every train needs to be added, I suppose you'll need to enter each train separately.
How can I manually add a train to my (existing) trip? I cannot find that option in the app.
