So I’m looking to plan for a train from Palermo to Naples and it lists all these stations. How do I know which to pick? I’m not really picky. Is there a setting where it can list ANY in the city?

Sure it is, it’s the first one. This is no station, it’s the city.
Which station depends on what is closest to where you'll be alighting from your previous train or where you're staying. The "City” options are not a fixed station but seem to represent a group of stations in that city.
