I don’t think they have a discount for disabled travellers, but they might be able to offer a companion pass, depending on your friend’s disability.

I don’t think so.
They might give you a free pass if you are her caregiver. I suggest you get in touch with Eurail directly. Use the link I posted above.
I don't know the exact rules about it.

We have a special offer for disabled travellers who need assistance during their travels: a complimentary Pass may be issued for the accompanying caregiver after submitting some requirements to our customer service team.
