I’ve got a Eurrail pass and I’ll be heading south from Lithuania and I’m not entirely clear how close I can get to it.

Sofia (Bulgaria). Then you can take a bus across the border. That’s the easiest way.

Otherwise: Bari/Brindisi (Italy) and then a ferry to Patras. Bus/train to Kiato, Athens and finally Thessaloniki.

I take it you don’t recommend Patras in Bulgaria

The closest you can get to completely by train is Kulata at the greek border.

But then there is a gap of 17 km to get to Thessaloniki (next station in Greece is Neo Petritsi). But I don't know if it makes sense to go that way.

Best way to go with Interrail is indeed a ferry from Italy to Greece (Patras) and from there via Athenes to Thessaloniki.
