Do you have an Eurail/Interrail pass? You can reserve seats in a Trenitalia train between Milano Centrale and Roma Termini on raileurope.com or buy the reservations at the ticket office.
For the metro in Milan and Rome, you need separate tickets. In Milan you can open the gates with your credit card. I don’t know it it is also possible in Rome or if you must buy a ticket.
