I am planning to visit 3 countries. Due to some personal problems i have to come back to my city after visiting first country and start to next country one day later. I stay in munich, Germany.
example— munich to italy
Italy to munich
then munich to france
France to Belgium
Belgium to munich
Can i take 5 days pass for my trip?

You would need for two journeys a individual ticket as you only have one inbound and one outbount journey in your homecountry germany :/
I would say best for the rides to/from Italy as the Austrian border is not far and you could easily take a direct train to Innsbruck (via Garmisch) or Kufstein and catch there then the Eurocity´s to Italy :)
Please note that for the Eurocity Germany - Austria - Italy a surcharge of 10€ applies for Railpassuser in 2nd Class or 15€ in 1st class (The surcharge is no reservation :/ )
