I have another question regarding the documentation.
Let's say, I want to travel from city A to city C, but have to stop and change in city B.
Do I now have to fill in: from A to C OR do I have to fill in: from A to B AND from B to C ?

You have to write down all the trains you take on your paper pass.
So you have to write down, from point A to point B. And from point B to point C.
