Are subways and buses included in "CONTINUOUS Eurail passes" for transit only within urban centers? In other words, would the passes also work for subways and buses in urban centers?

No, in general not valid on subways and buses (except rail replacement buses), even not for necessary transit. You need to buy a separate ticket.
