With the 6 days Greek island pass can i reserve a ticket for my car too on Ferries?
Is there a fee to pay?

The Greek Island passes are not intended for car drivers. So a car will have to be paid for separately and I doubt if the pass will then be of any use.
Even more strongly: you canNOT go with a car-its for ´foot´px only-just read a little further in the rules
