As it's not possible to do online reservations of Spanish trains, once you have made a reservation by phone or at a station, how do you physically add it onto the mobile app, and show it to the ticket inspector on the train? Thanks.

App/pass is completely separate from reservations, there is no way to add them.
You show your pass as this is your ticket and your reservation or whichever of these any checker wishes to see.
If you have bought at ticket office it will be on paper so show this, if bought online it will most likely be PDF so print out or have ready to display on device as well as pass.
