My wife purchased Eurorail pass for the both of us.  Do I need to create a separate account to access?

You don't need a Eurail account to use the pass. If you bought mobile passes, then you need to install the Rail Planner app and load the pass number your wife received. You don't need to activate the pass though before you start your trip.

For reservations, use this page as a guide:

You cannot transfer any unused days. You can only use them yourself.

Can I get a refund for unused days

That is not possible either.

You can only get a refund or do an exchange of an unused (not activated) pass.
